Job Responsibilities:
-Conduct baseline assessment of assigned community care services (nursing homes, eldercare centres or home care) to identify good practices and areas for
-Work with community care partners to develop nursing protocols and provide guidance on implementation of good clinical practices
-Provide inputs related to the practice of nursing where necessary
-Drive and facilitate Quality Improvement (QI) projects, initiatives and workshops aimed at organizational improvement for community care providers and within
-Apply and customize QI methodologies to suit the needs of the providers and to achieve the desired objectives
-Initiate and manage sector-wide events and platforms to build and sustain the quality improvement culture in the sector
-Promote successful improvements through the creation and/or delivery of presentations, articles, conference posters, etc
Job Requirements:
-Degree in Nursing with at least 8 years of relevant experience in nursing, project management and quality improvement
-Experience in quality improvement methodologies (e.g. LEAN, Plan-Do-Check-Act, Root Cause Analysis, Six Sigma Green/Black Belt) would be an advantage
-Good track record in facilitating quality improvement projects or programmed with good outcomes
-Experience and familiarity with the community care sector would be an advantage.
-Good project management, communication and analytical skills
-Independent, proactive and self-motivated, with the ability to adapt well to a fast-paced and dynamic environment
-Team player with great interpersonal skills
