1 Community Outreach and eye screening 50%

·      Make plans for community/house to house visit on weekly and monthly basis to get approval from supervisors.

·      Conduct meetings with authorities, group of people, family members in communities to explain purpose of his/her visit.

·      Make house to house visits to meet family members to recognize whether they have any eye problems.

·      Make appropriate registration and record keeping of individual examined for eye problems in a standard format/register provided with.

·      Record visual acuity without pinhole and with pinhole. Identify possible problems (such as cataract, pterygium, conjunctivitis) and refers the cases to eye unit (s) with referral form for further examination and treatment.

2 Create awareness and provide information on basic eye care 15%

·      Provide information on basic eye care and availability of eye care services at eye unit (s).

·      Counsel of patients for early treatment, referral and follow up at eye unit (s).

3   Refer patients to the partner hospitals and service 10%

·      Encourage patients to pay for their services at eye unit (s).

·      Coordinate and inform eye unit (s) team and Seva for referral of cases.

·      Act on meeting the target provided by Seva Office/Supervisor.

·      Follow-up with patient to get the surgeries and eye care services at partner hospital

4    Roll out the outreach Apps 20%

·      Recording the patients and screening details on real time.

·      Prepare reports on a regular basis.

·      Collect the stories and report the success stories to the supervisor. 

Any other responsibilities as agreed between the supervisor and supervisee 5%

·  Attend meetings as required by Seva

·  Provide general assistance to supervisor and Seva staff.

Skills and Attributes

The successful candidate will have 

• Minimum an associate degree, preference to candidate bachelor's degree
• Three years of work experience in community work or coordination or program Implementation in Cambodia or  Siem Reap province
• Strong interpersonal skills.
• Understanding of reading and writing English
• Ability on communication and outreach activities
• Tech literate and familiar with Microsoft Office (Word, Excel, PowerPoint).
• Ability to prioritize and work under pressure
• Working experience with government hospitals
• Aligns with Seva values
• Learning attitude 
• To be able to drive bike and travel in rural areas
